WHAT ARE THE CRITERIA FOR ADMISSION TO HOME HEALTH CARE?

In order to be admitted into home health care, the patient must be homebound, in need of health care that requires skilled nursing or physical, occupational, speech, or respiratory therapy on an intermittent basis, and referred by a physician, who must sign the initial plan of care to commence services.



WHO USES HOME HEALTH CARE?

Home health care can assist people of all ages, from children to senior citizens. Whether you have been discharged from a hospital or nursing home, and require additional care, or need assistance to remain at home, or personal care training, home health care is the solution for you.



WHAT TYPE OF PROFESSIONALS WILL BE AVAILABLE TO ME WITHIN HOME HEALTH CARE?

Our home health care team is comprised of referring physicians, registered nurses, licensed practical nurses, home health aides, licensed therapists (physical, occupational, speech and respiratory), and licensed medical social workers.



WHO PAYS FOR HOME HEALTH CARE SERVICES?

Home health care is covered by Medicare, Medicaid, and private insurance. We also accept private pay. Each insurance policy is different, so it is important to check in advance what services are covered. Ask us.
